Case managers link eligible clients with health care, mental health care and other services in a timely and coordinated manner. They can assist clients with Ryan White (Title II), AIDS Drug Assistance Program (ADAP), and more.

Case managers:

  • Assess a client's needs and personal support systems
  • Develop a service plan
  • Coordinate services to start the plan
  • Monitor and periodically reevaluate/revise the plan
  • Provide one-on-one education
