Case managers link eligible clients with health care, mental health care and other services in a timely and coordinated manner. They can assist clients with Ryan White (Title II), AIDS Drug Assistance Program (ADAP), and more.
Case managers:
- Assess a client's needs and personal support systems
- Develop a service plan
- Coordinate services to start the plan
- Monitor and periodically reevaluate/revise the plan
- Provide one-on-one education
